Simplifying Complex Restorative/Orthodontic Treatments through Continual Re-evaluation by Interdisciplinary Team and Patient
SYNOPSIS:
Most complex restorative and orthodontic patient treatments involve improper tooth position from tooth migration. Various dental conditions cause poor tooth position such as absence of teeth; poor tooth form or size from attrition, erosion, or inadequate restorations; and faulty genetic instruction of the teeth or bone.
When a patient needs restorative dentistry and the restorative dentist identifies these other oral problems, he or she often is tempted to correct all these problems solely using a restorative approach, thinking that the teeth need to be prepared anyway. This approach shuffles interarch and intra-arch space problems onto the technician, and concessions are forced on the restored tooth morphology due to compromised space and its effect on the intrinsic proportions of the teeth, the relationship of the occlusal to the incisal plane, and the interincisal angle.
A major difficulty for the restorative dentist, orthodontist and patient is visualizing the final shape and size of teeth with deficient tooth structure. To better visualize the final result, as space is created during orthodontic therapy, correct tooth form is restored with transitional restorations (bonding or properly contoured provisional restorations) and missing teeth are replaced with implants having properly sized and shaped provisional restorations. Results not previously envisioned may allow the treatment plan to be modified to eliminate the need for more costly treatment and still attain the desired
restoration.
Dr. Forbes and Dr. Vence, working as an interdisciplinary team, will demonstrate their collaborative approach through their ongoing reevaluation and continual dialogue with the patient. The benefit of this approach is a committed patient with realistic expectations, less treatment, less invasive restorations, reduced costs, and the ability to phase treatment with a more ideal esthetic and functional outcome.
DAVID FORBES BIOGRAPHIC SKETCH:
David Forbes graduated magna cum laude from University of Maryland Dental School in 1977. He received a Masters of Science in Orthodontics and a PhD in Oral Biology from Northwestern University where, in 1981, he was appointed program director of Orthodontics. He held joint appointments in the departments of Oral Biology, Orthodontics, Cell and Molecular Biology, and the Cleft Palate Institute. In 1986 he was promoted to full professor with tenure, and in 1987 he became chairman of Orthodontics and Co-Director of the Cleft Palate Institute.
Dr. Forbes has published in the American Association of Dentofacial Orthopedics, the Journal of Craniofacial Genetics and Developmental Biology, the Cleft Palate Journal, Journal of Craniomandibular Practice, and the Compendium. He is a co-author of Understanding Orthodontics. In 1993 he devoted himself full-time to his private practice of Orthodontics.
He is a past president of the Illinois Society of Ortho-dontists and member of the Am. Assoc. of Orthodontists, Am. College of Dentists, Am. Cell Biology, Am. Assoc. of Bone Research, American Assoc. of Developmental Biology, and Am. Academy of Restorative Dentistry.
BRIAN VENCE BIOGRAPHIC SKETCH:
Brian Vence maintains a private practice in West Dundee, Illinois, with a special interest in comprehensive esthetic and restorative dentistry. He graduated with a degree in honors biology in 1981 from the University of Illinois, Champaign-Urbana. He received his dental degree in 1985 from the University of Illinois School of Dentistry. He completed a hospital residency at the VA Wadsworth-UCLA in 1986 and a mini-residency in temporo-mandibular joint disorders.
Dr. Vence founded the Chicago Academy of Interdisciplinary Dentofacial Therapy in 1992, a study club designed to define and promote excellence in dentistry. He has had manuscripts published in Quintessence of Dental Technology, Practical Periodontics and Aesthetic Dentistry and The Journal of Prosthetic Dentistry and Compendium. He is an editor in fixed prosthodontics for Practical Procedures and Aesthetic Dentistry.
He is a member of the American Academy of Restorative Dentistry, the American Academy of Esthetic Dentistry, the International College of Dentists, and the American College of Dentists.
