MEMBERSHIP APPLICATION
The fee for new members is $3,000 for the first (calendar) year. Each year thereafter, the fee is $2,000 for the year's eight meetings. This fee also includes a three-course gourmet luncheon with wines at all meetings and a subscription to The Journal of Prosthetic Dentistry at a greatly reduced cost.
Photographs will be taken during the May meeting for the membership roster (the last meeting prior to the printing of the membership roster in August). If you miss an opportunity to have your photograph taken, please e-mail a current photograph to NHAD@hughes.net.
The fee for Guest Registration for any scientific meeting is $475. This fee may be applied to a new membership for the current year. Reservations for guests must be received one week in advance. Please do not phone. Once your guest fee is received, you will be registered to attend that meeting. Mail the guest fee with your name as you would like it to appear on your badge and your email address to the above address.
